Please note I no longer work at Wyndham Village Medical, MyClinic Werribee Village, nor Tarneit Doctors, I am now working full-time at Lancaster Medical Centre in Werribee. If you want to continue your care with me, please register here by clicking the book now button and selecting new patient when asked what appointment type you need. To transfer your previous records, visit Lancaster Medical Centre reception and complete a patient consent form, which will then be faxed to your last practice. Please note that the earlier practices may charge a record transfer fee. If your previous practice uses Best Practice, please ask them to email the records in XML format.
